Determining Optimal Integration of Hospital In-Patient and Ambulatory Care Coordination Programs

Many hospital systems across the country are concerned about their case management departmental program design and are considering relaunching the right case management program to match the patient population they serve.  Hospitals are focusing on strategically developing high quality hospital and ambulatory systems.  While many hospitals have successfully launched patient centered medical homes in their ambulatory practices, there remain multiple issues unresolved following discharge.  This causes patients to be readmitted.  Additionally, inpatient and ambulatory care strategic planning has often failed to produce the desired results related to care coordination and transitions of care goals.
